This book on breast diseases covers topics that will be of interest to clinicians and specialists as well as investigators. All topics are covered well, with largely excellent illustrative material, diagrams, and tables. Chapters that may be of particular interest are those on stem cells and gene arrays. The previous edition was published in 2004, thus the current edition is quite timely. This fourth edition continues to advocate a multidisciplinary approach for breast cancer as critical for achieving the best outcomes. For example, chapters detail concepts such as sentinel lymph node biopsy and breast conservation as well as mastectomy and more sophisticated modes of radiation. By analyzing all aspects of breast diseases, this book is certainly an excellent resource for clinicians as well as investigators. According to the authors, the audience includes both clinicians and translational investigators. This is an excellent resource for practitioners, residents, or fellows in training aspiring to be specialists in breast diseases as well as translational investigators. It might not be appropriate for students, due to the depth of information (including detailed information about ongoing clinical trials which can be tedious and cumbersome for students). The book summarizes well the various aspects of benign and malignant breast diseases. It covers anatomy, the clinical examination, radiology, the pathological basis of breast cancer, various treatment modalities including surgery, breast conserving approaches, neoadjuvant, adjuvant, new modalities in delivering radiation therapy, metastatic breast cancer treatment, hormonal therapies, receptors modulation as well biological and prognostic markers, with special attention to the molecular characterization of breast cancers, stem cells, and gene arrays. The multidisciplinary approach and the special scenarios like cancer in pregnancy and minorities are highlights. The breast cancer genomics and diagrammatic representations are very useful. The discussion of the pathology and biology of premalignant breast disease does an excellent job of explaining the spectrum of DCIS, and the pictures of histological and molecular diversity of DCIS are great. Precise summaries at the end of every chapter provide clear explanations of the current level of practice. It is hard to find flaws in this book, but it might have been worthwhile to include more surgical pictures showing operative techniques for mastectomy as was done well in the sentinel lymph node biopsy chapter. Also useful would be a final table analyzing all the nongenetic factors of breast cancer, although the authors do a great job of discussing the strength of association with breast cancer and possible strategies for primary prevention. Breast cancer is a fast-growing specialty with many recent advances in diagnosis and management, and the authors do an excellent job of integrating the various modalities of the latest approaches in explaining the pathologic basis of breast cancer, intense screening modalities in diagnosis which in turn reduce mortality, management which has changed significantly over the past few years, and the multidisciplinary approach which reduces morbidity. Many topics deserve special mention, especially breast-conserving surgery, changes in criteria for mastectomy, the role of biological markers, and new localized radiation therapy, as well prognostic factors, oncotyping, and new advances in stem cells, gene arrays, receptors modulation, etc. This fourth edition is an excellent resource and it certainly justifies replacing the previous editions.
